How do I begin services with your company?

  

If you are already receiving DD waiver services or wish to start services with us, you have the following options: visit our contact page or go to the homepage, scroll to the bottom, and book your free consultation. Together, we will create a strategic plan that aligns with your goals and the DBHDS vision for your loved one's development. After that, contact your local Support Coordinator for further guidance, and they will assess your loved one's needs. Use this website as a resource, and be sure to provide them with our NPI number: 1548004278.

How do I know if I need Community Engagement or Community Coaching?

Contact your local or assigned Support Coordinator. They are well-informed and well-equipped to assess the needs of your loved one and guide you through the process of starting services. .

Can I have multiple DD Waiver Services If I'm already enrolled?

 All requirements, and inclusions are determined based on the individual's needs. It is possible to be allocated multiple DD waiver services simultaneously. Please reach out to your local or assigned Support Coordinator to allow them to perform a full assessment. 

Are your services available on the weekends?

 Yes, we are available to work on weekends; however, our business hours are Monday through Friday, 9:00 AM to 6:00 PM. Each individual is allocated a certain number of service hours by their Support Coordinator, which must be reassessed and approved as needed. Medicare/Medicaid may limit the number of hours for various services, so please reach out to your Support Coordinator for further guidance on qualifying for additional hours.
